Considerations for personalized design and choice of material in single implant restorations
The development of CAD/CAM workflow was in a key role when gold-casted metalceramic restorations lost their popularity as material for implant restorations. Individual titanium and zirconia implant abutments were the first step in a new era of high accuracy, aesthetics, and strength, together with cheaper manufacturing methods. In clinical studies however, the individual zirconia abutments showed high failure rates. Additionally, the manufacturing of such individual abutments is often centralized which was prolonging the treatment time.
The development of prefabricated titanium base abutments enabled fully digital workflow for producing implant-supported restorations, however, long-term clinical evidence of these restorations does not exist yet. Questions have also been raised about the emergence profile of titanium base abutments possibly leading to biological problems. In the future, the key to success could be a fully personalized abutment-free implant restoration.
In this lecture the pros and cons of diOerent abutment / restoration combinations for single implant restorations are discussed in light of scientific evidence and clinical examples.
